Prior to Admission

Contact Forest Haven for a facility tour. You may call and ask for the Administrator, Assistant Administrator, or Admissions Coordination. Any of these individuals will be happy to set up a time for a tour that meets your convenience. If you decide that Forest Haven is right for you, then here's what to expect in the admission process.

If the individual to be placed at Forest Haven is in another nursing facility or at a hospital, a member of the Forest Haven nursing team will go to that facility to evaluate the individual's health care needs. this is os that we can be certain that our facility can meet the resident's health care needs.

If not in the hospital, the lady or gentleman to be admitted to the facility must visit his or her physician to have the paperwork for Nursing Home placement completed. These forms are called the 90-L and the PASARR. Physicians may have these forms, but it is best to stop by the office at Forest Haven and pick up the blank forms prior to going to the physician.

The facility will also need a copy of the individual's medical history and physical exam ("H" & "P").

Additionally, a PPD skin test must be administered and a chest x-ray must be performed (with a negative result for TB).

A Forest Haven representative will contact the family member, responsible party or hospital social worker when the resident has been approved for admission to Forest Haven.

On The Day of Admission

Select clothing and other items to be brought to the facility. Please mark all clothing with laundry proof ink.

New resident is transported to the facility. Forest Haven or your hospital social worker (if person is in the hospital) will assist you in arranging transportation, if he or she must be transported by ambulance. Personal items are moved into the room and the resident is made comfortable in his or her room.

Family member or responsible party completes admission interview with Forest Haven staff (usually our admissions coordinator). This will usually take no more than 45 minutes to 1 hour.

The family or responsible party (or ambulance attendants, if person was transported by ambulance) brings the 90-L and PASARR, the PPD and chest x-ray, the medical history and physical, and the resident's social security card, private insurance cards, and medicare card to the facility. If the individual is already receiving Medicaid benefits, a copy of the Medicaid benefit card or information is needed.

Checklist of Personal Items for New Residents

I. Clothing: An adequate supply of 5 to 7 days is recommended.

  • dresses or gowns, loose fitting
  • warm-ups or fleece suits
  • shirts
  • socks
  • undergarments
  • house shoes or slippers (2 pair)
  • walking shoes
  • night clothes
Laundry services are provided at no extra charge by Forest Haven staff. However, if family members choose to launder resident's clothing, a sealable container is suggested.

II. Grooming

  • comb, brushes
  • toothbrush, toothpaste
  • body lotion
  • deodorant
  • shaving lotion, etc. for men
III. Room Decorations
  • pictures or photographs for walls, if desired
    (we ask that you let our maintenance department hang these)
  • plants or flowers
  • books, magazines, etc...
  • small lamp for table or floor
  • table or wall clock
IV. Furniture, if desired
  • recliner or favorite rocker or chair
  • small chest of drawers
  • small refrigerator
  • small television
    (Forest Haven provides basic cable)
V. Telephone, if desired
  • Residents may have a telephone installed in their room. Forest Haven staff will assist as needed. However, telephone are the responsibility of residents. Responsible parties may contact Bell South at 1-557-6500 to request telephone service.
